Materials needed to compile the envelope:

MONO + permanent eraser

Dot Instrument (I used my Cricut Instrument)

Mechanical pencil MONO Graphics

Ruler (again Scream)

Scissors

Step 1: Cut the paper into an 8-inch square

Start by cutting the paper into 8 inch squares. Then mark the center (4 inches) on each of the four sides.

Then use the ruler to combine the 2 marks. Then use the notes tool to draw a questionnaire. Repeat this process on the opposite side.
